Epididymitis Treatment in Birmingham

Epididymitis is inflammation of the epididymis, the coiled tube at the back of the testicle. It is a common cause of scrotal pain in men of all ages. Mr Syed Ali Shahzad, Consultant Urological and Robotic Surgeon, assesses and manages epididymitis at private hospitals across Birmingham.

Causes

In men under 35, epididymitis is most commonly caused by sexually transmitted infections (STIs), particularly chlamydia and gonorrhoea. In older men, it is more often caused by urinary tract bacteria (such as E. coli) associated with underlying urological conditions such as BPH. Occasionally it may be associated with viral infection (mumps orchitis) or non-infectious inflammatory causes.

Symptoms

  • Pain and swelling in one or both testicles, developing over days
  • Tenderness over the epididymis (the back of the testicle)
  • Fever and general malaise in some cases
  • Urinary symptoms (frequency, burning) when associated with UTI
  • Urethral discharge if caused by an STI

Diagnosis

Scrotal ultrasound with Doppler confirms epididymitis (showing increased blood flow) and excludes testicular torsion, which is a surgical emergency requiring immediate attention. Urine culture and STI screening (urethral swab or first-catch urine PCR) guide antibiotic choice.

Treatment

Epididymitis is treated with antibiotics, choice depending on the likely causative organism. Pain is managed with anti-inflammatory medication and scrotal support. Underlying urological problems are treated once the acute episode has settled.
