Nocturia Treatment in Birmingham
Nocturia is the need to wake from sleep to urinate on two or more occasions per night. It is one of the most common and troublesome lower urinary tract symptoms, affecting sleep quality, energy levels, daytime functioning and overall quality of life. Causes of Nocturia
Nocturia has multiple potential causes and often requires a systematic assessment:
- Nocturnal polyuria: The kidneys produce an excessive proportion of the daily urine output at night. This is the most common cause in adults over 65 and may be related to heart failure, sleep apnoea, fluid redistribution from the legs, or reduced antidiuretic hormone secretion.
- Reduced bladder capacity: Due to overactive bladder, interstitial cystitis, or previous bladder surgery.
- Bladder outflow obstruction: BPH or urethral stricture causing incomplete emptying and increased voiding frequency.
- Global polyuria: Excessive urine production throughout the day and night, due to conditions such as diabetes mellitus or diabetes insipidus, or excessive fluid intake.
- Medications: Diuretics taken in the afternoon or evening, lithium, or excessive caffeine and alcohol.
Assessment
Mr Shahzad uses a frequency-volume diary (completed over at least 3 days), urine tests, flow rate and post-void residual measurement, and where appropriate urodynamics, to identify the precise cause of nocturia before recommending treatment.
Treatment
Treatment is directed at the identified cause. Options include fluid management advice, afternoon/evening diuretic restriction, medication adjustment, desmopressin (for nocturnal polyuria), treatment of OAB with Botox injections or medication, and treatment of prostatic obstruction.
