Chronic Prostatitis Treatment in Birmingham
Chronic prostatitis and chronic pelvic pain syndrome (CPPS) is one of the most prevalent and challenging urological conditions, affecting up to 15% of men. It is characterised by persistent pelvic, perineal or genital pain, often with urinary and sexual symptoms, lasting more than 3 months. Mr Syed Ali Shahzad, Consultant Urological and Robotic Surgeon, offers a structured, evidence-based approach to chronic prostatitis at private hospitals across Birmingham.
Symptoms
- Persistent or recurring pain in the perineum, lower abdomen, penis, testicles or lower back
- Pain during or after ejaculation (often the most distinctive symptom)
- Urinary frequency, urgency, slow flow or incomplete emptying
- Erectile dysfunction
- Reduced libido
Causes
The exact cause of CPPS remains incompletely understood. Factors that may contribute include previous infection, pelvic floor dysfunction, nerve sensitisation, psychological stress and lifestyle factors. In most cases, no ongoing bacterial infection is identified.
Diagnosis
A thorough clinical assessment, symptom questionnaires (NIH-CPSI), urine cultures and imaging help to categorise the condition and exclude other causes such as prostate cancer, bladder cancer or interstitial cystitis.
Treatment
A multimodal approach is most effective:
- Alpha-blockers to relax the bladder neck and prostate
- Anti-inflammatory medication
- Pelvic floor physiotherapy
- Neuromodulation or nerve blocks in selected cases
- Psychological support and cognitive behavioural therapy
- Lifestyle modifications including dietary changes and stress management
