MRI of the Prostate in Birmingham

Multi-parametric MRI (mpMRI) of the prostate is now the recommended first-line investigation for men with a raised PSA or clinical suspicion of prostate cancer. What Is Multi-Parametric Prostate MRI?

Multi-parametric MRI combines several different MRI sequences (T2-weighted, diffusion-weighted and dynamic contrast-enhanced imaging) to provide a detailed picture of the prostate’s internal architecture. It can identify suspicious areas, characterise their likely significance, assess the size and location of lesions, and detect whether cancer has spread beyond the prostate capsule or to local lymph nodes.

The PIRADS Classification

Radiologists report prostate MRI findings using the PIRADS (Prostate Imaging Reporting and Data System) scoring system, from 1 (most likely benign) to 5 (most likely clinically significant cancer). A PIRADS 3 or above score generally warrants targeted biopsy to clarify the diagnosis.

Why MRI Before Biopsy?

NICE guidelines recommend performing an mpMRI before biopsy in men with raised PSA. The “MRI-first” approach:

  • Identifies clinically significant cancers more accurately
  • Allows targeted (rather than random) biopsy of suspicious lesions
  • Reduces unnecessary biopsies in men with no suspicious lesion
  • Avoids detection of clinically insignificant low-grade cancers that do not require treatment

What to Expect

The scan takes approximately 30 to 45 minutes and does not require any specific preparation, though an enema may occasionally be recommended. Results are reported by a specialist uroradiologist and discussed with you at a follow-up consultation with Mr Shahzad, who will advise on whether a biopsy is required.
