Types of Prostatitis
- Acute bacterial prostatitis: A sudden onset infection of the prostate, causing fever, rigors, severe perineal or pelvic pain, and urinary symptoms. This is a medical emergency requiring prompt antibiotic treatment.
- Chronic bacterial prostatitis: Recurrent urinary tract infections caused by persistent bacterial infection of the prostate. Requires prolonged antibiotic treatment.
- Chronic prostatitis / Chronic pelvic pain syndrome (CPPS): The most common form. Characterised by pelvic, perineal or genital pain without a clear bacterial cause. Management is complex and multidisciplinary.
- Asymptomatic inflammatory prostatitis: Found incidentally on biopsy or investigation; causes no symptoms and generally requires no treatment.
Symptoms
Symptoms vary by type but may include pelvic, perineal or lower back pain, pain during or after ejaculation, burning or pain on urination, urinary frequency and urgency, and occasionally fever and flu-like symptoms in acute cases.
Diagnosis
Assessment includes urine culture, PSA testing, digital rectal examination and, in some cases, further imaging or cystoscopy to exclude other causes of symptoms.
Treatment
Bacterial prostatitis is treated with targeted antibiotics. Chronic pelvic pain syndrome is managed with a combination of alpha-blockers, anti-inflammatory agents, pelvic floor physiotherapy and, in some cases, psychological support.
