Bladder Instillation Therapy in Birmingham

Bladder instillation is a treatment in which a therapeutic fluid is introduced directly into the bladder via a thin catheter. It is used for the treatment of bladder cancer and interstitial cystitis / bladder pain syndrome. Mr Syed Ali Shahzad, Consultant Urological and Robotic Surgeon, provides bladder instillation therapy at private hospitals across Birmingham.

Bladder Instillation for Bladder Cancer

Following TURBT, intravesical (into the bladder) chemotherapy or immunotherapy is instilled to destroy any remaining cancer cells and reduce the risk of recurrence:

  • Mitomycin C: A chemotherapy agent instilled immediately after TURBT or as a course of weekly treatments for low- to intermediate-risk non-muscle-invasive bladder cancer.
  • BCG (Bacillus Calmette-Guerin): An immunotherapy agent instilled weekly for 6 weeks, used for high-risk non-muscle-invasive bladder cancer. BCG treatment significantly reduces recurrence and progression rates.

Bladder Instillation for Interstitial Cystitis

For patients with interstitial cystitis or recurrent UTIs, instillation with protective agents can reduce bladder symptoms:

  • Hyaluronic acid (Cystistat): Replenishes the protective glycosaminoglycan (GAG) layer of the bladder lining.
  • Heparin or lidocaine combinations: Reduce bladder inflammation and pain.

The Procedure

Bladder instillation is a straightforward outpatient procedure performed without sedation in most cases. A thin catheter is passed into the bladder, the solution is instilled, and the catheter is removed. The fluid is retained in the bladder for a defined period before being voided.
