Hydronephrosis Treatment in Birmingham
Hydronephrosis occurs when urine builds up in the kidney, causing it to swell. It is not a disease in itself but a consequence of an obstruction or problem affecting the normal flow of urine. Causes of Hydronephrosis
Common causes include:
- Kidney stones blocking the ureter
- Pelvi-ureteric junction (PUJ) obstruction
- Enlarged prostate causing urinary retention
- Tumours pressing on the ureter
- Obstructive uropathy
- Pregnancy (can cause physiological hydronephrosis)
- Urethral stricture
Symptoms
Hydronephrosis may be painless, particularly if it develops slowly. When symptoms occur, they include flank pain, nausea, fever (if infection is present), changes in urinary output, and blood in the urine.
Diagnosis
An ultrasound scan is usually the first investigation. CT urogram or MRI provides more detailed information about the degree of obstruction and its cause. Functional studies (such as a MAG3 renogram) assess how well each kidney is working and whether intervention is needed urgently.
Treatment
Treatment is directed at the underlying cause. Temporary drainage using a ureteric stent or nephrostomy may be required to protect the kidney while the cause is investigated. Definitive treatment depends on the cause and may include stone removal via ureteroscopy, or surgical correction of a PUJ obstruction with robotic pyeloplasty.
